Inspector’s narrative

What the inspector wrote

On Monday, 22 September, 2025, at approximately 11:30 AM Licensing Program Analyst (LPA) Fabian Schwartz met with Director Mireya Torrez to open and close a Complaint Investigation. At time of inspection, there was a census of 28 preschool children being supervised by 6 staff and the director. It was alleged that a child sustained injuries at the facility due to staff's lack of supervision. Throughout the course of the investigation, LPA conducted interviews, reviewed documents, and made observations. Throughout the investigation, evidence gathered did not produce a preponderance of evidence to support the allegation. Although the alleged violation may have happened or is valid, the preponderance of evidence standard has not been met to fully prove or disprove that they did or did not occur, therefore, the allegation is unsubstantiated. An exit interview was conducted and a notice of site visit and appeal rights were provided. Notice of site visit shall remain posted for 30 days.
