What the inspector wrote

On Friday, May 15,2026, Licensing Program Analyst (LPA) Sala Vang met with Kalei Walsh for the purpose of an unannounced required annual inspection. Upon arrival, LPA observed a census of 11 children supervised by 4 staff. Licensee Representative was reminded never to exceed the condition, limitations and capacity specified on the license. All individuals subject to criminal background review have obtained a criminal record clearance. . Facility hours of operation are Monday through Friday, 08:00 AM to 05:30 PM LPA toured classroom space and outdoor play area. LPA observed the following documents are posted: License, Parents' Rights Poster, Seat Belt Law, Menu, Daily Schedule, Commingling Waiver, and Insufficient Outdoor Space Waiver. Technical Assistant was advise about Lead Posting. LPA observed a functional carbon monoxide detector and full 2A10BC fire extinguisher. LPA observed electronic sign in/out system. LPA observed a children’s roster and fire drill log. Facility has record of conducting fire drills at least every six months. The last fire drill was conducted on April 4, 2026. Cleaning disinfectants and hazardous items are appropriately stored and inaccessible to children. Director stated there is no poison or firearms on the premises. LPA advised the applicant that all poisons must be locked with a key lock or combination lock. Furniture and equipment are in good condition, and toileting facilities are in safe, sanitary, and operating condition. LPA observed trash bins with tight fitted lids. LPA observed labeled food and milk for infants, stored at safe temperatures. LPA observed staff documentation of feedings and diaper changes. Page 1.....Continue 809-C LPA observed a current Needs and Services plan for each infant and an Infant Sleeping Plan (LIC9227) for each child under 12 months of age. LPA observed sleep checks being conducted and documented every 15 minutes for all children under two. LPA observed that all cribs were free of loose articles and objects and there was one crib or mat for each child in care. LPA observed each crib was equipped with a clean tight-fitting sheet. Staff stated that the bedding is changed daily. LPA observed that the Infant diaper changing area is clean and within arm's reach of a sink. Changing table contains a sanitary vinyl surface mat that is at least one-inch-thick with raised sides. LPA reviewed present staff files. At least one staff member present today has current Pediatric CPR/First Aid certification which expires on 2/2028. All staff currently present and currently employed with the facility have: a criminal record clearance, a health screening report, immunization records, current mandate reporter training, and documentation of their educational background, training, and/or experience. LPA advised AB1207 Mandate Reporter training must be renewed every two years and can be renewed at www.mandatereporterca.com/california . LPA reviewed files and found that only one employee is a fully qualified teacher. Facility has two infant classroom in operation today. A sample of 5 children’s records were reviewed. Each child’s files contained an emergency card, consent for medical treatment, health history, a record of immunization's, and notification or personal and parent right. LPA observed a current Needs and Services Plan for each infant and an Infant Sleeping Plan (LIC9227) for each child under 12 months of age. LPA observed sleep checks conducted and documented every 15 minutes for all children under two. LPA observe infants sleeping in a sleep sack and sleep sacks were by all cribs. LPA explain to Director Janika that sleep sacks are not allowed in childcare. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5 years after the date of the first test as specified in Health and Safety Code section 1597.16. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). Page 2.....Continue 809-C LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred licensee representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES. Licensee representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee representative of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA Vang informed licensee representative Janika that this report dated 5/15/2026 document’s 2 Type A citation’s which shall be posted for 30 consecutive days as there is/are immediate risk’s to the health, safety, or personal rights of children in care. Page 3.....Continue 809-C Also, LPA Vang informed the licensee Janika Walsh provide a copy of this licensing report dated 5/14/2026 that documents any Type A citation’s to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Licensee representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Representative, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
