What Happened

Three-year required inspection found multiple compliance issues including exposed hazards on outdoor play structures, staff documentation gaps, and incomplete mandated reporter training. Deficiencies being cited.

Observations

  • three wood panels located under patio shaded area with sharp nails sticking out accessible to children - director stated panels will be removed before children present in back play area
  • wear and tear observed on cushioning material around high climbing equipment - director advised to replace or repair if it becomes a tripping hazard
  • two staff missing licensing required documents
  • one staff had not fully completed AB1207 required mandated reporter training
