What the inspector wrote

On 11/5/24, Licensing Program Analyst (LPA) Priscilla Zamudio conducted a complaint inspection at the facility for the purpose of delivering the findings to the above listed allegations. LPA met with Assistant Director, Carol Gallegos Gonzalez, toured the facility and took a census. During the course of the investigation, LPA Zamudio conducted interviews with the reporting party, staff and parents. Interviews and LPA observations reflect children were in a chaotic environment, climbing on tables and unanchored play furniture, multiple times. It was also revealed that the classroom was messy and unorganized with blankets and other items on the floor. Based upon the information gathered and interviews conducted, this agency determined that the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 12, Chapter 1, deficiencies will be cited (see 9099-D). Exit interview conducted with the Assistant Director, Carol Gallegos Gonzalez. A copy of this report and Appeal Rights were provided and discussed. A Notice of Site Visit (LIC 9213) form will be posted on the facility's parent's board and must remain posted for 30 days.

This posed a potential risk to the health and safety of the children in care.
