What the inspector wrote

On 08/19/2025, Licensing Program Analyst (LPA) Aurelio Mendoza conducted an unannounced Case Management \342\200\223 Incident inspection at the facility. LPA met with Director Tasha Avalos, who accompanied LPA during a tour of the facility both inside and outside. LPA explained the purpose of the inspection was to follow up on an incident self reported to the Child Care Licensing Regional Office on 08/11/2025 regarding inappropriate behavior by a daycare child. Per incident details, child #1 was observed by staff #1 using a Velcro strap, typically used to secure children\342\200\231s artwork puzzles, as a sling toy while the classroom curriculum activity was taking place. Staff stated that child #1 has a pattern of not showing interest in curriculum activities; however, staff #1 allows children to quietly engage in independent activities that do not distract the rest of the class. Staff observed the strap being used to sling toward the wall, which later escalated into use with a ball as a slingshot. At that point, staff #1 intervened to ensure the health and safety of all children in care. At approximately 10:30 a.m., staff #1 directed child #1 to return the strap to its designated area. Upon retrieving the strap, the child stated they had sustained a scratch. Upon inspection and review, no bruises were found; however, staff reported observing a small dot sized mark, which they stated may have occurred while retrieving the strap from the child after the child was told multiple times to return it to its designated area. Records reviewed did not indicate staff were out of ratio at the time of the incident. Staff reported using redirection strategies such as distraction, changing activities, and sitting with the child to address listening challenges. Documentation reviewed by LPA confirmed transparency with the parent of child #1 and showed ongoing one on one efforts by staff to support the child\342\200\231s needs and ensure safety. During the inspection, LPA interviewed staff, made classroom observations, and reviewed facility documents. Interviews and documentation revealed the classroom was adequately staffed, with staff present and engaged in curriculum activities in close proximity to children when the incident occurred. Although the incident occurred, investigation findings did not show staff handled the situation inappropriately. Interviews revealed that staff communicated the incident to the parents of the child involved. Furthermore, interviews and documentation indicated staff maintain ongoing communication and collaboration with child #1\342\200\231s parents and continue working to mitigate challenging behaviors to prevent further incidents. Although the incident was confirmed, LPA determined it was an isolated incident. Staff responded appropriately, followed required policies and regulations, and reported the matter to licensing as required. Per Title 22, Division 12, of the California Code of Regulations, no deficiencies are cited. An exit interview was conducted with Director Tasha Avalos. This report is to be made available to the public upon request. LIC 9213 Notice of Site Visit to be posted for 30 days.
