What the inspector wrote

On 10/03/2025, Licensing Program Analyst (LPA) Meche Rosales conducted an unannounced case management inspection. LPA met with Director Tasha Avalos, toured the facility, and took a census. The purpose of today\342\200\231s inspection was to discuss an incident that was self reported to Community Care Licensing (CCL) on 08/28/2025. Per incident details, children were picked up at school and transported to the facility for after-school care. During the ride, Child #1 showed the \342\200\234shiv\342\200\235 to other children being transported, and upon arrival, they informed staff #1 about what child #1 had showed them on the ride to the facility and showed other children in the classroom. Director stated it looked like a pen that had been broken up into pieces. The teacher told child #1 to put the item away in his backpack. The teacher informed director, who confiscated the item. Before the items were confiscated, director stated that child #1 had threatened another day care child with the \342\200\234shiv\342\200\235 in the classroom. director talked to child #1, who said he made it because he wanted protection from people who would attack him at places outside of care, such as a \342\200\234baseball game.\342\200\235 During the inspection, LPA made classroom observations, and reviewed facility documents. LPA discussed supervision at the time of the incident, and it was determined that appropriate supervision was in place. Furthermore, LPA did review a copy of the email correspondence between parent and director, a picture of the \342\200\234shiv,\342\200\235 as well as a copy of the progressive guidance action plan that has been placed in child #1\342\200\231s file signed by the parent. LPA determined staff took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements. An exit interview was conducted, and the report was reviewed with Director Tasha Avalos. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, there are no deficiencies being cited during today\342\200\231s inspection. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
