What the inspector wrote

On 01/23/2026, Licensing Program Analyst (LPA) Meche Rosales and Licensing Program Manager (LPM) Joseph Pacheco met with Director Tasha Avalos for an unannounced case management inspection. A complete file review was conducted prior to today's inspection. LPA toured the facility, and a census was taken. The purpose of today's inspection was to address reporting unusual incidents and the facility\342\200\231s plan of operation. During today\342\200\231s inspection, LPA and LPM discussed with Director multiple unusual incidents that have occurred in recent months involving the same two children having inappropriate behavior towards day care staff and day care children. LPA and LPM discussed and reviewed facility\342\200\231s Parent Handbook and Admission Agreement with Director and reviewed applicable Title 22 regulations. LPA and LPM also discussed requirements for reporting Unusual Incidents to Community Care Licensing. The director stated that moving forward, reporting requirements and admission agreements will be followed. Per California Code of Regulations Title 22, Division 12, Chapter 1, no deficiency was cited during today's visit. An exit interview was conducted with Director Tasha Avalos. A notice of site visit and appeal rights was given.
