What the inspector wrote

On June 10, 2025, Licensing Program Analyst (LPA) Julio Rodriguez conducted an unannounced Case Management Inspection. LPA met with Director Tasha Avalos. LPA toured the facility, and a census was taken. The purpose of today's inspection was to gather more information regarding an Unusual Incident that was self-reported to the Fresno Childcare Regional Office, the incident occurred on 05/07/2025. The incident involved a child possibly sustaining an injury while at the center that required medical attention. During today’s visit, LPA Rodriguez conducted facility observations and interviewed staff. LPA observed during the visit that the preschool classroom was staffed appropriately, and staff were observed interacting and attending to children in care. Staff interviewed indicated they observed children playing rough in the classroom on the day the alleged injury occurred and stated that they had attempted to intervene through redirection. Although a child reported being kicked by another child during the rough playing, staff indicated they did not observe any injuries or signs of distress on the child the remainder of the day prior to pick up time. Staff interviewed indicated that the classroom was appropriately staffed on the day of the incident. Staff interviewed indicated they practice proper supervision and are aware of teacher-child ratio requirements per department regulations. During today’s inspection LPA Rodriguez observed the facility practiced proper supervision while children are playing inside the classroom. Furthermore, LPA determined it was an isolated incident and staff took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements. Teacher/child ratios are being maintained, and adequate supervision is being provided during today's visit. During today's inspection, LPA Rodriguez observed the child involved in the incident was present and actively participating in the classroom. Continued on 809-C. Exit interview was conducted and report was reviewed with Director Tasha Avalos. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, there are no deficiencies being cited during today’s inspection. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
