Licensing Program Analyst (LPA), Meche Rosales, conducted an unannounced case management inspection. The purpose of the visit was to meet with Director Tasha Avalos. LPA served her with a Letter of Exclusion for a staff member for all three licenses. Director indicated she did not receive a notification from the HR Dept, but also director stated the mailing address was incorrect. The director ensured that the staff member is immediately excluded from having contact with children and from being physically present at the facility. Director has been advised this exclusion action remains in place, unless otherwise notified in writing by the Department/Guardian. Director was presented with a form LIC 300A, the Confirmation of Removal, and director completed the form. LPA will return form to CCLD and place it in the office file for this facility. The director stated she understood the process of background clearance and she stated that future employees without fingerprint verification shall not attend any training’s, nor do they enter the facility at any time. Based on the evidence obtained during today’s visit, LPA has verified the individual is not present. Verification of removal is complete. Per Title 22, Division 12, no deficiencies cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit was given and will be posted for 30 days. Appeal rights were also provided.
