Inspector’s narrative

What the inspector wrote

Based on the investigation, the preponderance of evidence has been met, that staff are not providing adequate food service to daycare children and that staff did not notify parents of accurate incidents regarding their children; therefore, the above allegations are found to be SUBSTANTIATED. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, the following deficiencies are being cited during today’s inspection. (See next page LIC9099-D). Marie was provided a copy of appeal rights. Exit interview conducted and report was reviewed with Marie. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiency is being cited during today’s inspection. Marie was provided a copy of appeal rights.     (f) The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative.This requirement is not met as evidenced by: During the complaint investigation, facility records were reviewed and records documented that a child sustained only one injury; however, records submitted to the facility by the child’s authorized representative showed child sustained multiple injuries and staff failed to disclose to the child’s authorized representative. This posed a potential health, safety or personal rights risk to persons in care.

Each meal shall include, at a minimum, the amount of food components as specified...This requirement is not met as evidenced by: During the complaint investigation, it was revealed that the food served to the children was not in quality and in quantity to meet the necessary needs of the children in care. This posed a potential health, safety or personal rights risk to persons in care.
