(f) The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative.This requirement is not met as evidenced by: During the complaint investigation, facility records were reviewed and records documented that a child sustained only one injury; however, records submitted to the facility by the child’s authorized representative showed child sustained multiple injuries and staff failed to disclose to the child’s authorized representative. This posed a potential health, safety or personal rights risk to persons in care.
