What the inspector wrote

On 6/22/23, Licensing Program Analyst (LPA) Priscilla Zamudio conducted an unannounced case management inspection. LPA met with Acting Site Supervisor, Graciela Banda to discuss an incident which occurred on 6/2/23 and a census was taken. LPA interviewed staff and observed the area in which the incident occurred. On 6/2/23, at approximately 9:30am, there were 8 children with 3 staff in the outdoor area. Staff were setting up for a graduation to begin at 10:00am and two staff also went inside for break. At approximately 9:35am, one of the Teachers who had the day off but was there for her child’s graduation, was outside then walked into the classroom from the backdoor. She spoke to staff who were in the breakroom inside the classroom but then returned outdoors. Staff who were on break stated that within seconds, the office assistant walked into the classroom from the front door and informed them that a child was observed in the classroom. She walked the child back outside and notified the Acting Site Supervisor. The staff inside the breakroom stated they never saw the child in the classroom as the breakroom door was closed. Acting Site Supervisor called the child’s parent to inform them of the incident and the parent mentioned that the child sneaks away in public places. Acting Site Supervisor stated this was the first time a child walked away in the facility, and they have taken extra steps to ensure this does not occur again. Based on information gathered and observations made by LPA, there were no obvious concerns or hazards found. Staff are implementing adequate care and supervision at the facility. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. Exit interview conducted and report was reviewed with Acting Site Supervisor Graciela Banda. This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
