Based on record review, the licensee did not comply with the section cited above in that testing was conducted on 02/23/2023. LPA was provided copies of BSK Associates Laboratory Fresno. LPA observed that there are no exceedances. Yet, licensee did not have licensing forms or images on file for LPA to review, which poses/posed a potential health, safety or personal rights risk to persons in care.
