What the inspector wrote

On 04/25/2025, a Case Management Inspection was conducted by Licensing Program Analyst (LPA) Nohemi Sanchez. LPA met with Lead Teacher Romana Camp to discuss an incident report submitted to Community Care Licensing regarding an incident that occurred at the facility on 03/20/2025. Lead Teacher stated that on the date of the incident, Child #1 was climbing up the slide while two other children were sliding down. This resulted in a collision and a pile-up involving Child #1. The other two children landed on top of Child #1, causing an injury to the child\342\200\231s arm. Teacher #1 applied an ice pack and administered first aid. Teacher #2 then escorted Child #1 to the nurse\342\200\231s office where it was advised that child be taken to get a medical evaluation. The parent was notified, and after picking up Child #1, took the child to the emergency room for evaluation. It was determined that Child #1 had sustained a fractured arm. Child #1 returned to care on 03/25/2025. Teacher #1 and Teacher #3 witnessed the incident and had previously reminded the children that one child should slide down at a time. At the time of the incident, Teacher #2 was positioned in between the slides on the play structure when a child called for assistance. As Teacher #2 responded, the incident occurred. Both Teacher #1 and Teacher #2 immediately went to assist Child #1. Continued on LIC809-C. Teachers will continue to remind children that they should not climb up the slide to prevent similar incidents and avoid injury. Staff will also monitor and consistently advise children that only one child should slide down to avoid collisions. LPA inspected the play structure where the incident occurred and did not observe any safety hazards or areas of concern. This appears to be an isolated incident, and Teachers took appropriate measures to address the child\342\200\231s injury, following appropriate policies, regulations, and report requirements. Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency was cited during today\342\200\231s inspection.
