What the inspector wrote

On this date 7/6/2023, Licensing Program Analysts (LPAs) Ruby Ocegueda and Meche Rosales conducted an unannounced case management \342\200\223 unusual incident inspection. LPAs met with Site Supervisor Irene Carillo, toured the facility, and took a census. The purpose of today's inspection was to follow-up on an unusual incident that was reported to Community Care Licensing (CCL) on 6/14/2023. The incident reported was regarding child #1 (who is in classroom #1). Child #1 fell and bumped his/her head while climbing on the metal play structure causing a small open wound to his/her head. Parent did seek medical attention and child returned to the facility the next day without incident. During today's inspection, LPA Ocegueda interviewed staff regarding the unusual incident, reviewed personnel files, inspected facility and facility equipment. LPA discussed supervision and interventions implemented since the incident. LPAs observed the play structure was intact, with no broken or sharp areas and there was enough sand in fall zones. Per Site Supervisor, the sand is tilled regularly to avoid any hardening of the sand. Per staff interviews, child to teacher ratio requirements were met on that day. There were 21 children in care in room #1 with two fully qualified teachers (teacher #1 and teacher #2) supervising the children during outside play. The center has a policy of 8:1 teachers supervising children, however on this occasion teacher #3 was on his/her break. LPA advised to continue to discuss outside play safety with children and to continue with the practice of reduced child to teacher ratio per the facilities policy. Today, it was determined that the facility met all reporting requirements as specified in Title 22 Regulation 101212 - Reporting Requirements. Facility submitted all required information to CCL. Today, it was determined that the incident is an isolated incident and not a result of lack of care and supervision. No deficiencies cited in the areas observed today.
