What Happened
Case management inspection investigated serious incident on 07/03/2023 where two children engaged in inappropriate sexual conduct while under insufficient supervision during nap time. Staff member had left children unattended to turn on lights, resulting in Type A violation.
Observations
- On 07/03/2023 during nap time, two children were left in quiet area under watch of Teacher Carrillo
- Teacher Carrillo walked away for approximately 2 minutes to turn on lights for other children
- Both children found covered under blanket when teacher returned
- Child #1 had removed his pants and underwear
- Child #2 was engaged in inappropriate sexual behavior with Child #1
- Type A violation issued for inadequate supervision

Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time.This requirement was not met as evidenced by: LPA's obervation. Supervision of children shall include visual observation and children should not be left unattended.This is a potential personal rights, health and safety risk to children in care.
