What the inspector wrote

Licensing Program Analyst (LPA) Sha Fee Vang and LPA Aurelio Mendoza conducted an unannounced Case Management \342\200\223 Incident inspection. LPAs met with Director Sabrina Corral, toured the facility, took a census, and explained that the purpose of the visit was to follow up on a self-reported Unusual Incident from 01/12/2026. The incident involved a child who fell while dismounting a tricycle in the outdoor play area and was later diagnosed with an elbow fracture. Staff reported the incident occurred at approximately 11:25 a.m. while 15 children were outside with two supervising teachers. Staff stated they were in proximity, observed the fall, responded immediately with first aid, and notified the authorized representative. The child received medical care and returned to care the next day wearing a sling. Based on interviews and observations, LPAs determined that required teacher-child ratios and supervision were maintained at the time of the incident. Staff described the use of outdoor zoning to ensure continuous visual supervision. LPAs observed the outdoor environment and tricycles to be age-appropriate and within regulatory guidelines. Information indicates the incident was isolated and not due to lack of supervision. No deficiencies were cited today under Title 22, Division 12, Chapter 1. An exit interview was conducted with Director Sabrina Corral. LIC 9213 (Notice of Site Visit) was issued and must remain posted for 30 days. LIC 9058 (Appeal Rights) was provided.
