What the inspector wrote

On 05/20/2025, Licensing Program Analysts (LPAs) Elizabeth Martinez and Octavia Nolan conducted an unannounced case management visit and met with Director Elena Elizondo. LPAs took a census and a tour of the facility. During today’s inspection, LPA interviewed the Director and staff. On 05/08/2025, the facility notified Community Care Licensing (CCL) via telephone regarding an unusual incident. On 05/07/2025, during transition to lunch, a child was briefly left in the restroom alone. After conducting the case management inspection and interviewing staff, this appears to be an isolated incident. The facility has taken necessary measures to prevent further incidents by installing half doors and a staff meeting regarding care and supervision. The Director understands that no child should be left without visual supervision of a teacher at any time. The facility took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies were cited today. An exit interview was conducted with Director, Elena Elizondo. LIC 9213 Notice of Site Visit was provided to Director and will be posted for 30 days.
