What the inspector wrote

On 05/20/2025, Licensing Program Analysts (LPAs) Elizabeth Martinez and Octavia Nolan conducted an unannounced case management visit and met with Director Elena Elizondo. LPAs took a census and a tour of the facility. During today’s inspection, LPA interviewed staff. On 05/09/2025, the facility notified Community Care Licensing (CCL) via telephone regarding an unusual incident. On 05/08/2025, at 1:00 PM, Classroom B was playing on the outdoor activity area. Child #1 (C1) needed to use the restroom and was escorted to the restroom by Staff #1 (S1). S1 asked Staff #2 (S2) to return C1 to the outdoor play area. While walking outside, S2 became distracted and C1 walked into class B, which was empty by herself. After conducting the case management inspection and interviewing staff, this incident appears to be a result of a lack of care and supervision and C1’s health and safety could have been adversely impacted. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see LIC 9099-D). An exit interview was conducted with Director, Elena Elizondo. LIC 9213 Notice of Site Visit was provided to Director and will be posted for 30 days.
