What Happened
Unannounced case management follow-up on incident from 5/8/2025 where a child wandered unsupervised into an empty classroom when staff became distracted. Deficiency cited for lack of supervision.
Observations
- Child #1 walked into empty Classroom B unattended while staff distracted
- Staff #2 became distracted during walk and failed to return child to outdoor area
