What the inspector wrote

On 09/18/2025, Licensing Program Manager (LPM), Kari Mc Williams and Licensing Program Analyst (LPA), Yesenia Fierro conducted a Case Management-Incident inspection. LPA met with Directors Shelbi Ture and Colleen Mackey. LPA explained the reason for the inspection was to follow up on an unusual incident report. LPM and LPA toured the facility, and a census was taken. On August 29, 2025, the Fresno Regional Office received an Unusual Incident Report indicating that on the same day, Child #1 (C1) was observed crying before nap time. After waking from the nap, C1 continued to cry and complained of pain in their left arm. C1 was taken to a medical provider for evaluation. At approximately 5:45 PM, Director Ruiz followed up with C1’s father, who informed her that C1 had been diagnosed with a greenstick fracture. During today’s inspection, LPM and LPA conducted interviews with staff. Interviews conducted revealed there was not enough evidence to determine how the injury to C1’s arm occurred. Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency is being cited Exit interview conducted and report was reviewed with the Director Shebi True A notice of site visit was given and must remain posted for 30 days.
