What the inspector wrote

Based on interviews and records reviewed during the investigation, the allegation that licensee does not maintain adequate records of day-care children is corroborated. The preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 12, Chapter 3, a Type B deficiency is being cited on the attached LIC 9099-D. An exit interview was conducted with Licensee Audelia Rios. A copy of this report and Appeal Rights were provided and discussed with Licensee Audelia Rios. A Notice of Site Visit Form was provided and must remain posted for 30 days.

    IMMUNIZATIONS

    (g) The licensee shall document each child's immunizations as required by the California Code of Regulations, Title 17, Section 6070, and shall maintain such documentation for as long as the child is enrolled.This requirement is not met as evidenced by: Based on interviews and record reviews, Licensee did not have immunization records for 11 out of the 14 children that are enrolled in the facility. The Licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

  • ALTERATIONS TO EXISTING BUILDING OR GROUNDS

    (a) Prior to making alterations or additions to a family child care home or grounds, the licensee shall notify the Department of the proposed changed, including, but not limited to, the following: (6) Any change from an area of the family child care home previously identified as "off limits" to an area where... care and supervision will be provided to children in care.This requirement is not met as evidenced by:Based on LPA's observations, interviews and records review. The daycare storage room and master bedroom were to be “off limits” and were not permitted for childcare use, which poses a potential health, safety or personal rights risk to persons in care.

  • OPERATION OF A FAMILY CHILD CARE HOME

    (g) The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not limited to: (10) A baby walker shall not be allowed on the premises of a family child care home in accordance with Health and Safety Code Sections... 1596.846(b) and (c).This requirement is not met as evidenced by:Based on observation, the licensee did not comply with the section cited above in that an infant was observed in a baby bouncer which posed a potential health, safety or personal rights risk to persons in care.
