Inspector’s narrative

What the inspector wrote

On 08/07/2025, Licensing Program Analysts (LPAs) Miguel Herrera and David Rocha conducted a Case Management - Plan of Correction inspection. LPA met with Licensee Audelia Rios, who accompanied LPAs on a tour of the facility. Licensee is Spanish Speaking and LPA Herrera assisted with interpretation. Present during today\342\200\231s inspection were 7 children. The purpose of today\342\200\231s inspection was to verify that the deficiencies issued during the inspections on 07/28/2025 have been corrected. On 07/28/2025, the facility was issued three (3) Type B citations in violation of California Code of Regulations. CCR 102416.3(a)(6) - On today's inspection LPA Herrera verified that Licensee Rios made "off-limit rooms" inaccessible to children in care by locking doors and using door knob spinners. CCR 102417(g)(10) - On today's inspection, LPA Herrera verified that licensee removed bouncers and walkers from her home. Licensee's Plan of Correction for the above listed deficiencies have been cleared during the visit and Plan of Correction letter was provided to Licensee Rios. CCR 102418(g) - On today's inspection, Licensee Rios asked LPA Herrera to verify the files for all children as she believed that she had all immunization records on file for children in care. LPA Herrera verified children files and observed 13 children files with current immunization records on file. Licensee stated that the parent of child #1 and child #2 has not submitted immunization records for the children therefore she decided to exclude them from care until she obtains records. She stated that if she cannot get their records by 08/08/2025, she will ask for a POC extension. LPA Herrera understood. Per Title 22, Division 12, Chapter 3 of the California Code of Regulations, no deficiencies were observed today. Exit interview conducted with Licensee, Audelia Rios and appeal rights were provided. A notice of site visit (LIC 9213) was given and must remain posted for 30 days.
