What the inspector wrote

On 07/09/2025, Licensing Program Analyst (LPA) Xona Xayavong conducted an unannounced case management inspection and met with Licensee Dana Castillo Millan. LPA informed Licensee that the purpose of the visit was to follow up on additional information provided by the Licensee during a telephone call on 06/25/2025. Also present during the inspection was the Licensee\342\200\231s assistant. A tour of the facility was conducted, and a census was taken. On 06/25/2025 Licensee contacted LPA via telephone and provided LPA with a different name of the uncleared adult living in the home. Based on the additional information Licensee provided regarding the complaint investigation that was concluded on 06/24/2025, the violation is still valid and the POC is correct, LPA only required to update the LIC 421BG form. LPA corrected \342\200\234Individual in Violation #1\342\200\235 from \342\200\234Adult #2\342\200\235 to \342\200\234Adult #1.\342\200\235 LPA retrieved the LIC 421BG form originally printed on 06/24/2025 from the Licensee and issued a revised version reflecting the correction. Per Title 22, Division 12, Chapter 3 of the California Code of Regulations, no deficiencies were cited during today\342\200\231s inspection. An exit interview was conducted with Licensee Dana Castillo Millan. A copy of this report and the Appeal Rights were provided and reviewed with the Licensee. LIC 9213 Notice of Site Visit was issued and must be posted in a visible location for 30 days.
