What the inspector wrote

On 04/10/2026, Licensing Program Analysts (LPAs) Nohemi Sanchez and Kimberly Vass conducted an unannounced case management inspection. LPAs met with Director Martha Jimenez, toured the facility inside and outside, and took a census. LPAs informed the Director of the purpose of the inspection. On 04/06/2026, the Fresno South Regional Office received a report regarding an incident that occurred the same day. During an outdoor nature walk, Child #1 managed to walk away from the group and was observed on the roadway approximately five feet from the sidewalk. Staff #1 immediately retrieved the child. No injuries were reported. Following the incident, the nature walk ended and all staff and children returned to the facility. Staff notified the Director, and the Director immediately informed the child’s mother of the incident. According to the Director, all nature walks have been canceled for the near future, and any walks will be conducted within the fenced area of the facility. The Director will develop a plan of correction to prevent similar incidents from occurring. Staff were reminded to maintain constant supervision and to remain vigilant of their surroundings, particularly when supervising infants and toddlers during outdoor activities. This appears to be an isolated incident. Staff took appropriate action in response to the child’s incident and followed all applicable policies, regulations, and reporting requirements. The report was reviewed, and an exit interview was conducted with Director Martha Jimenez. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiency was cited during today’s inspection.
