On 4/27/2026, Licensing Program Analyst (LPA) Jose Ruiz and Licensing Program Manager (LPM) Scott Herring conducted an unannounced case management - incident inspection. LPA met with Licensee, Elizabeth Silva and a census was taken. LPA toured the inside and outside areas of the home. The purpose of the inspection was to follow up on the unusual incident report received by the Fresno Regional Office on 04/09/2026. On 4/08/2026, child #1 experienced a medical incident, which required medical attention. 911 and the child's parents were called. Child #1 was taken to the hospital by ambulance, where she was treated. Child #1 returned to the facility on 04/14/2026. During today’s inspection, LPA conducted record reviews and interviewed Licensee and her assistant. Interviews and records revealed that Licensee’s unusual incident report was a submitted timely. At this time further investigation it needed. An exit interview was conducted, and the report was reviewed with Licensee. Appeal rights were provided. Per Title 22, Division 12, Chapter 3 of the California Code of Regulations, no deficiency are being cited.
