What the inspector wrote

An unannounced case management inspection was conducted today at 08:36am by Licensing Program Analyst (LPA), Kayla Danielson. LPA met with licensee facility representative Martha Baker. In response to two Unusual Incident Report's received by the Department on 02/21/2025 and 03/04/2025. The incident on 02/21/2025 involved a child getting hurt on their upper lip playing on the outside ramp leading to the classroom resulting the child being taken to the doctor. The incident on 03/04/2025 involved a child hurting their nose while playing outside on a tree stump resulting in them needing medical attention. The facility representative Director Martha was interviewed on 03/20/2025 at 09:01am and stated that on 02/20/2025 at 12:30pm C1 wanted to play with one of the toy car and started to drive it around hunched over. C1 started to go fast with it resulting in them slipping near the outdoor ramp and hitting their upper lip on the ramp. C1's lip was bleeding and Martha cleaned it up then contacted their parent. Parent came and picked up the child to be seen by a doctor. Martha kept in contact with parent who relayed doctor cleared their child and they returned to school the next day. Director Martha stated on 02/26/2025 at 10:30am C2 was running with friends outside when they tripped and hit their nose on a tree stump in the play yard. C2's nose started bleeding and was helped by another staff member inside to start cleaning it up. Martha assisted in cleaning C2's nose while a staff member contacted a parent. Parent came to pick up child and took them to the doctor to be seen. Martha kept in contact with parent who relayed child was okay and would be returning to school on their next scheduled day. Director Martha relayed the staff are watching children more closely and keeping a closer eye on the ones who don't listen to the outside rules as well to help redirect and minimize accidents. The staff is also going over the rules for the outside play time with the children daily to help them minimize accidents and ensure the children are kept as safe as possible. 1 of staff (S1) was interviewed on 03/20/2025 and stated that on 02/26/2025 they were on their break with C2 was brought in with a bloody nose and assisted in cleaning her up. S1 stated C2 told them that they were outside playing with their friends and fell down. S1 contacted parents who came and picked up C2. 1 child (C2) was interviewed on 03/20/2025 and stated they were outside running playing with friends and hurt their nose with they fell down. During today\342\200\231s inspection, the facility was toured inside and out. LPA observed 15 children playing inside and outside without incident and being properly supervised. LPA observed children involved in snack time. There were no deficiencies cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with the facility representative Director Martha Baker. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
