What the inspector wrote

An unannounced case management inspection was conducted today at 9:26am by Licensing Program Analyst (LPA), Erica Laird. LPA met with licensee Deanna Hamilton in response to an Unusual Incident Report received by the Department on 6/3/25. Per the incident report, three children (C1-C3) were found to have exposed themselves and possibly touched one another in the genital region. The licensee was interviewed on 6/3/25 and 6/20/25 and stated on 5/28/25 at approximately 3:15pm three children (C1-C3) were on the play structure outside having snack. Another child (C4) went over to the play structure and observed C1-C3 acting suspicious. C4 told the assistant (S1) and licensee that C1-C3 were acting strange. Licensee stated she pulled C1 aside and asked what was going on and C1 stated they were playing "doctor". Licensee stated all three children were spoken to about appropriate behavior and private areas of the body. Licensee stated she spoke to all parents about the incident when the children were picked up later that day. Licensee stated she did not immediately report the incident to licensing because she did not feel the actions of the children were outside of normal developmental behavior and none of the children seemed upset. report continued on 809-C During today's inspection one staff (S1) was interviewed. S1 stated they were outside with the children when the incident occurred. S1 stated they did not witness the incident but they did have visibility of the children and the play structure. S1 stated the children did not seem upset by the incident. S1 stated there has not been previous incidents of a similar nature, that they are aware of. On 6/19/25 LPA Laird conducted interviews with two parents (P1-P2). Both parents interviewed stated they were aware of the incident. Both parents interviewed stated they had no concerns with the supervision being provided at the facility and felt the situation was handled appropriately. Both parents stated they had not heard of similar incidents happening at the facility in the past. During today\342\200\231s inspection, the facility was toured and 16 children records were reviewed. LPA observed licensee and assistant providing active supervision of all children present. LPA Laird observed the play structure and outside play area. LPA Laird observed the play yard has direct visibility to the play structure from every angle of the play yard, however, there are compartments of the play structure which contain blind spots. LPA Laird took 4 photos of the play structure and play yard. Based on interviews, the following deficiency is being cited on the LIC809-D. Reporting Requirements 102416.2(a)(3)(C). Exit interview conducted and report was reviewed with the licensee, Deanna Hamilton. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
