What the inspector wrote

On 10/24/25 @ 11:20am an unannounced case management inspection was conducted by Licensing Program Analyst (LPA), Erica Laird. LPA met with licensee, Deanna Hamilton in response to an Unusual Incident Report received by the Department on 10/09/25. Per the incident report, On 10/09/25 a child (C1) was standing near an open kitchen door when another child (C2) closed the door, shutting C1's finger in the door jam. C1 was taken by ambulance where it was determined C1 had lost their fingernail and a small portion of their left ring finger. On 10/24/25 @ 11:29am LPA Laird conducted an interview with licensee, Deanna Hamilton. Deanna stated on 10/09/25 a child had gone upstairs to grab art supplies. C1 was standing near the door and another child (C2) closed the door and C1's left ring finger was slammed in the door jam. Deanna stated C1's mom came and called 911. C1 was taken by ambulance. Deanna stated C1's lost their finger nail and a small portion of the skin. Deanna stated C1 did not get stitches and is back at the facility part time. During today's inspection LPA Laird toured the facility and took 2 photos of the door. LPA Laird conducted an interview with one assistant (S1). Based on interviews and observations, LPA Laird determined the incident was not a result of lack of supervision. There were no deficiencies cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with the licensee, Deanna Hamilton. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
