An unannounced case management inspection was conducted today 04/29/2025 at 3:30 p.m. by Licensing Program Analysts (LPA) Noah Wheeler and Kiriko Lynch. LPAs met with facility representative Betsy Wilson in response to an Unusual Incident Report received by the Department. Facility representative stated the classroom was transitioning from outside to inside and the staff miscounted during the transition. She stated a teacher (S1) coming back onto shift noticed two children (C1-C2) were on the playground, and immediately notified classroom staff. She stated staff immediately went outside and found C1 and C2 playing \342\200\234hide and seek\342\200\235, and both children were brought inside after approximately 7-8 minutes of being left unattended. The facility representative was interviewed on 02/12/2025, and stated that on 01/24/2025 at approximately 11:40 a.m., two children (C1-C2) were left outside during transition, and staff did not notice the children were missing for approximately 7-8 minutes. Four additional staff (S1, S3, S4) were also interviewed on 02/12/2025 and stated C1 and C2 were left unattended for approximately 7-8 minutes and (S2) stated the child was left unattended for approximately 8-10 minutes during transition from outside to inside right before lunch. Continued on 809-C During today\342\200\231s inspection, the facility was toured and LPAs observed care and supervision to children provided by staff to children in care. LPAs observed outside playground is completely fenced with a latched gate. Based on interviews and record review, LPAs found staff failed to conduct accurate head counts for children when transitioning from outside to inside, and staff also failed to check the playground for children who may have been hiding, and the following deficiency is being cited on the LIC809-D. LPAs Noah Wheeler and Kiriko Lynch informed facility representative Betsy Wilson that this report dated 04/29/2025 documents one Type A citation, which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. LPAs Noah Wheeler and Kiriko Lynch informed the facility representative to provide a copy of this licensing report dated 04/29/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in children's files for verification.
