LPA Lynch visited the center for the purpose of a follow-up case management visit regarding a self-reported incident initially reported on 04/11/25. Facility representatives stated a teacher grabbed a child by his collar to stop him from potentially injuring himself and others and child tripped and fell to the floor. Facility representatives provided written staff accounts to both Licensing and the local police department. It was revealed that staff witness accounts of the incident were all differing in details of the former lead teacher allegedly using inappropriate verbal and physical discipline for child in attendance. It was also revealed that the child did not sustain injuries from the incident and also trips and falls on his own regularly per witness interviews from the police report. Facility representatives stated they spoke with the parent immediately after the incident, and the child did not leave after the incident and remained in care. LPA also conducted interviews with facility representatives and parents, and no further disclosures were made regarding witness accounts of former lead teacher yelling or grabbing children. Based on interviews and records review, LPA found there was an inappropriate level of discipline by staff for a child at the center. The following deficiency is being cited on the LIC809-D. Exit interview conducted and report was reviewed with the Administrator Dannielle Petersen. Notice of site visit posted, and appeal rights were provided.
