On 10/22/2025, at 11:25am, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Case Management inspection to follow up on a self - reported incident. LPA met with Facility Representative, Andrea Jimenez and disclosed the purpose of the inspection. LPA accompanied by Facility Representative toured the indoor and outdoor of the facility. This is a full day program which operates on a traditional school year schedule. Days and hours of operation are Monday \342\200\223 Friday, 8:00am \342\200\223 2:30pm. There were 14 children present, with 4 staff members. Facility Representative, Sergio Serrano reported that on 10/6/2025 at approximately 4:30pm, he was advised of an alleged personal rights violation involving Child #1 (C1). LPA conducted interviews with staff members and C1. LPA reviewed staff and children files, obtained copies of facility roster, and related documentation. Facility Representative, Rachel Ramos will submit the sign in/sign out sheet, to LPA, no later than 10/29/2025. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Andrea Jimenez and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and the report was reviewed with the Facility Representative, Andrea Jimenez.
