What the inspector wrote

On 10/23/2025, at 10:30 am., Licensing Program Analysts (LPA) Raina Alexanian and Cindy Meier conducted an unannounced case management inspection to follow up on a self-reported unusual incident. LPAs met with facility director, Matilde Montes. LPAs discussed the purpose of the inspection and were led on a tour of the facility. There were sixteen (16) children present with four (4) staff members. On 09/30/2025, facility director reported to Community Care Licensing that on 09/23/2025, Child #1 (C1) went home early due to a right arm injury. C1 was evaluated and treated by a medical professional the same day. Mom returned to the facility with C1 to drop of the doctor note on 09/23/2025 with no restrictions. C1 did not stay at school due to classes ending. Facility staff speculate the injury occurred while C1 was on the playground; however, the specific cause is unknown. LPA interviewed staff and daycare parent. Based on interviews conducted and records reviewed, LPA determined that the facility was operating within ratio on the date of the incident, no personal rights violation occurred and reporting requirements were met. No citations were cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with the facility representative director Matilde Montes. A notice of site visit was given and must remain posted for 30 days.
