What the inspector wrote

On 02/4/2025 at 11:35am, Licensing Program Analyst (LPA), Selina Siao conducted an unannounced Case Management inspection for a decrease in capacity from 78 to 55 children ages 2 to 5 years old in rooms F and G. Facility is also extending their hours of operation to Monday to Friday from 7am to 10pm. Upon arrival, LPA met with Site Supervisor Isabel Garcia. Site Supervisor stated that the facility has not been operating since June of 2024 due to low enrollment. No children are in care today. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Ms. Isabel that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. LPA continued to tour the facility and remeasured the indoor and outdoor activity space. Total indoor activity space rooms F and G measured to be 1822.79 square feet, which is only sufficient to accommodate up to 52 children rather than the requested capacity of 55 children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via a Sparklett water dispenser that are in both rooms. LPA observed all hazardous items to be inaccessible to children in the classrooms. There are no bodies of water or weapons on the property. Fire clearance was granted on 1/3/2025. LPA observed a total of 6 sinks and 6 toilets available for children’s use. These are sufficient to accommodate the requested capacity. There is a separate staff’s restroom located in the office area that will also be used for sick children. The facility currently has a fully fenced playground area. The chain link is at least four feet high. The total square footage for all the outdoor activity space is 7898.40 square feet, which is sufficient to accommodate the requested capacity. Shade is provided via large shade tarps structures. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure and swings on the playground for children ages 2 to 12 years old which is properly anchored. There is adequate cushioning in fall zones of climber provided by soft rubber cushioning. Drinking water is available via igloo when children are outside with paper water cones provided by the facility. LPA observed all hazardous items on the playground to be inaccessible to children. Facility representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For child care center licenses shall test their water for lead every five years from the initial lead water testing date. The following corrections are needed prior to the issuance of the license: Add room letters on both sides of the doors for rooms F and G. Remove all cleaning items in the staff’s restroom as that is the isolation restroom for children. Update both parents board with updated licensing forms. Remove the two storage cabinet in both rooms. Submit an updated LIC500 personnel report. Submit an updated LIC200A application with requested capacity of 52 preschool children ages 2 to 5 years old due to indoor activity space. Facility representative Isabel Garcia understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
