What the inspector wrote

On 11/9/2021 at 10:19am, Licensing program Analyst (LPA) Martha Malane, conducted an unannounced Annual Inspection and met with Site Supervisor, George Vasquez. LPA disclosed the purpose of the inspection and was led on a tour of the facility indoors and outdoors. This is a full day program which operates on a traditional school year schedule. Days and hours of operation are Monday \342\200\223 Friday 8:00am \342\200\223 2:30pm. There are currently two (2) classrooms in operation, rooms 1 and 2 on the Phil Swing Elementary school campus. The following ratios were observed: Classroom 1 (serves children age 3 - 5 years): 13 children present with two (2) staff Classroom 2 (serves children age 3 - 5 years): 10 children present with two (2) staff Mr. Vasquez stated there is no swimming pool or other bodies of water on the premises. Mr. Vasquez stated there are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection. Furniture and equipment are in good condition. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment and slides have cushioning material to absorb falls.Toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. Kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Food is protected against contamination and contaminated food is discarded immediately. The facility is free of flies, insects and rodents. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. See LIC809C continuation... Employees are fingerprint cleared through the Imperial County Office of Education. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the child care center director or fully-qualified teacher designated to act in the director\342\200\231s absence was not reported to the Department. Site Supervisor, George Vasquez submitted required paperwork to LPA during today\342\200\231s inspection. Mr. Vasquez was reminded that the person who signs the child in/out of the facility shall use their full legal signature and record the time of day; Technical Assistance was provided. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles. Staff members 1 - 5 (S1 - S5) do not have current documentation of completed AB1207 Mandated Reporter training. Menus are posted at least one week in advance where an authorized representative can view them. This facility provides Incidental Medical Services (IMS). LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. Site Supervisor is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and site supervisor discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, California Megan\342\200\231s Law ( www.meganslaw.ca.gov ), Forms and Regulations. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page, LIC809D). An exit interview was conducted with Site Supervisor, George Vasquez. Mr. Vasquez was provided a copy of: LIC809, LIC809C, LIC809D, 9102TN, appeal rights (LIC 9058) and LIC 9213 Notice of Site Visit which is required to be posted for 30 days.
