Inspector’s narrative

What the inspector wrote

On 01/23/2023 at 11:07 a.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a pre-licensing inspection. Upon arrival LPA, met with Coordinator, Jean-Mari Dagarin and proceeded to tour the facility. The facility is requested a capacity of 24 preschool children; serving ages 3 through 5 years old in classroom #214. Classroom #214 is located in building 200. The program will operate at Sea View Elementary. Facility plans to operate following the school districts school calendar: Monday, Tuesday, Thursday and Friday from 8:00 a.m. through 2:30 p.m. and Wednesday from 8:00 a.m. through 2:00 p.m. Program is title 5. The classroom was inspected and measured during this inspection and measured a total of 1,119.18 square feet, which is sufficient to accommodate 31 children at one time. However, facility is only requesting a capacity of 24 children. The classroom was observed to be equipped with age appropriate furniture and play equipment; including napping equipment (mats), tables, chairs, cubbies, bookshelves, and other materials for the children use. The classroom is orderly, free of hazards and has adequate heating, lighting and ventilation. There is an open corridor to classroom #214 \342\200\223 Transitional Kindergarten near the shared restrooms. Plan of operation shall be submitted to ensure supervision is maintained and there is no co-mingling of the children. There is one sink in the classroom and two restrooms; each containing one sink and one toilet for the children\342\200\231s use. The restroom will be shared by the transitional kindergarten program, classroom #215. The number of toilets and sinks is sufficient to accommodate the requested capacity of 24 children. The restroom and hand washing areas are in a safe, sanitary and operating condition, per observation. The school district will provide breakfast, lunch and AM and PM snacks for the children. Uncontaminated drinking water is readily available during indoor and outdoor activities through the use of refillable water pitchers and disposable cups provided by the facility or cups brought from home. The nurse\342\200\231s office will be used as an isolation area for ill children. Also, the nurse\342\200\231s restroom located in the nurse\342\200\231s office will be utilized as an restroom for ill children. There is a functioning carbon monoxide detector and a first aid kit with required supplies located in the classroom, per observation. Disinfectants, cleaning solutions, and other hazardous substances shall be kept inaccessible to the children in care. The facility will be utilizing manual sign in and sign out. Required documents were observed to be posted near the front entrance of the classroom. The outdoor preschool play area was inspected and measured a total of 25,609.27 square feet which is sufficient to accommodate 341 children at one time. The outdoor play area is fully fenced. The classroom has direct access to the outdoor play area. There is a sandbox in the outdoor play area. LPA informed applicant representative, the sandbox shall be inspected daily and kept free of hazardous foreign materials. There is a play structure located in the outdoor play area. LPA is unable to determine suitable ages for the play structure. There is sufficient rubber matting material for cushioning under and around the play structure that absorbs falls; including swings and a slide. LPA observed adequate shade in the outdoor play area by the use of a large canopy over the play structure. The outdoor play area will be shared by the transitional kindergarten program. LPA observed the outdoor activity space surfaces to be free of hazards. There is no evidence of rodent or insect activity, per observation. Proper supervision and placement of staff during outdoor activities was advised to ensure the supervision and the safety of children during outdoor play. LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. This facility plans to provide Incidental Medical Services \342\200\223 IMS. IMS plan on file and approved. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA informed applicant initial water lead testing results shall be received and posted within 180 days of licensure. LPA provided and discussed the following with the applicant: PIN\342\200\231s 21-04-CCP, 22-06-CCP and 21-21-CCP, External Water Sampler Self-Certification Form (LIC 9275), Child Care Center Sampling Checklist Form (LIC 9276). The following is pending with a due date of January 27, 2023: \302\267 Playground waiver request due to shared outdoor space. \302\267 Restroom waiver request due to shared restroom usage. \302\267 Plan of operation due to open corridor to classroom #214 (Transitional Kindergarten). A fire clearance for 24 Preschool children; ages 3 through 5 years old in classroom #214 was granted on January 24, 2023. An exit interview was conducted with Coordinator, Jean-Mari Dagarin. The following reports were discussed and provided: LIC809 pages and Appeal Rights (LIC9058). Once pending documents, after review and approval of waivers a preschool license will be issued for 24 children; ages 3 through 5 years old in classroom #214. Once license is issued the license will be mailed to the licensee and once received shall be posted in the facility.
