What the inspector wrote

On 7/18/2025 at 12:15 PM, Licensing Program Analyst (LPA) Angela Nguyen conducted an unannounced Case Management inspection to follow-up on a self-reported incident that occurred on or about 05/30/2025. LPA disclosed the purpose of the inspection and was granted a tour into the facility by the Licensee, Graciela Chavez. Present in the home was Licensee's son Brian Chavez and (1) day care child. The incident that occurred on or about 5/30/2025 was regarding Child #1, (C1) wandered away from the facility. Interviews were conducted with the Licensee and Licensee's son. Licensee stated during nap time, she was in the bathroom when C1 went out of the front door onto the drive way. Licensee stated she heard the alarm on the front door go off when approximately 1 minute passed before she made contact with C1. Licensee's son observed the incident from the kitchen window and corroborates that C1 did not make it onto the street or sidewalk and the Licensee made contact with the child on the driveway. Licensee stated she informed C1's authorized representative of the incident the day it occurred. Based on review and interviews, the Licensee responded in a timely manner and has precautionary measures in place. Licensee stated she has installed an additional lock at the top of the door. No Deficiencies cited during the inspection. A Notice of Site Visit (LIC 9213) was given to Licensee, Graciela Chavez and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and the report was reviewed with Licensee, Graciela Chavez.
