On March 11, 2026, at 1:31 PM Licensing Program Analysts (LPAs) Angela Nguyen and Saray Siqueiros- Frayre conducted an unannounced case management inspection to follow up on a self reported injury reported on March 6, 2026. LPAs met with the Licensee, Sandra Cabrera and led on a tour of the facility. There were three (3) children and Karley Lopez were present during the inspection. One of those children was an infant. It was reported that on March 5, 2026 while dropping off children at school, a 2-year-old child was sliding down a slide and flipped, hitting his head leaving a mark on his right eye. Licensee stated that she was standing at the bottom of the slide at the large structure in the public park next to the school when the incident happened. Licensee stated that the injury was not reported to the child\342\200\231s parents. Licensee stated that as of today, the child has not receive any medical attention. Notice of Site visit was given and must be posted for 30 days. Exit interview was conducted and this report was reviewed with the licensee, Sandra Cabrera.
