What the inspector wrote

On April 29, 2026, at 11:21 a.m., Licensing Program Analysts (LPAs) Angela Nguyen and Saray Siqueiros-Frayre conducted a case management visit to follow up on a self reported injury reported on March 6, 2026 and to deliver an amended report created on March 11, 2026. LPAs met with Licensee, Sandra Cabrera and were granted entry into the facility. LPAs were led on a tour of the facility by the Licensee. There were 4 children and licensee's daughter present during the inspection. 2 of those children were an infant. It was reported that on March 5, 2026, a child sustained an injury at a public park prior to drop off at school. Licensee admitted that she did not comply with the requirement to report the child’s injury to the authorized representative. California Code of Regulations, (Title 22, Division 12 & Chapter 3) one type B deficiency, is being cited on the attached LIC 809-D. A technical advisory notice was issued. Notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and this report was reviewed in Spanish with Licensee, Sandra Cabrera by LPA Saray Siqueiros-Frayre.
