What the inspector wrote

On Friday, December 12, 2025, Licensing Program Analyst (LPA) Hanna Cha met with Site Supervisor Katti Johnson and Early Learning Program Coordinator Peggy Lindsay to conduct an unannounced Annual/Random inspection of the facility. LPA disclosed the purpose of the inspection to the Site Supervisor and toured the facility. At time of arrival, LPA observed three teachers and one teacher assistant providing care and supervision to nine preschool-aged children. The current days and hours of operation are Mondays to Fridays from 8:00 a.m. to 2:15 p.m. Physical Plant/Indoor/Children\342\200\231s Area: The facility operates out of one classroom. The facility includes two children restrooms, one kitchenette, and an outdoor play area. The preschool classroom was observed for age-appropriate toys/equipment, books, and supplies for children. LPA observed individual cubbies labeled with children\342\200\231s names and sufficient napping cots. The cleaning compounds are stored in high cabinet in kitchenette, stored separately from food and made inaccessible to children by a child safety lock. The sign-in and sign-out sheets were observed. Per Site Supervisor, smoking is prohibited on the premises. Per Site Supervisor, there are no firearms/weapons stored on premises. The disaster drills last completed on 12/12/2025. The carbon monoxide alarm and smoke alarm were observed. Per Site Supervisor, the lead testing for water last conducted in 02/2023. Bathroom: The two childcare bathrooms were observed for safe and sanitary operating conditions with no hazardous materials. Toilet paper, paper towels, and soap are available to children. The toilets and sinks are in operable condition. Kitchenette: LPA observed the kitchenette for safe and sanitary conditions. The kitchenette includes a refrigerator/freezer, sink, food preparation counters, and pantries. The knives and sharp items stored in high cabinet, out of reach of children. LPA observed a half door that separates the kitchenette from the classroom. Per Site Supervisor, the children are provided meals from the school cafeteria. The kitchenette is only used to prepare snacks. Outdoor/Children\342\200\231s Play Area: The outdoor play areas were inspected for safety, cleanliness, good repair, age-appropriate toys/equipment, cushioned floors, and shady areas. Per LPA observation, there are no pools or spas on the premises. File Review: LPA reviewed two children files which contained all required documentation. LPA reviewed child file currently on prescription medication. LPA verified LIC 9221 and doctor\342\200\231s note with instructions on file. LPA reviewed Site Supervisor\342\200\231s file and two staff files. Per interview and record review, Staff #1 (S1) was not fingerprint cleared on Guardian. The Early Learning Program Coordinator, Peggy Lindsay also verified that S1 was not listed on the facility roster. LPA conducted a Person Search on Guardian and did not identify S1 on Guardian. Per interview with Early Learning Program Coordinator, S1 is a substitute for the district. However, S1 did not get fingerprint clearance for child care. Per Early Learning Program Coordinator, S1 was present at facility on 12/11/2025 and 12/12/2025 for a total of two days. During the inspection, S1 immediately left facility and submitted fingerprints. LPA informed Early Learning Program Coordinator that S1 may not return to facility until fingerprints are cleared. A Type A citation issued with a Plan of Correction (POC) due 12/13/2025 and civil penalty of $200 assessed ($100 per day). Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred licensee facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP.
