On 04/23/2026, Licensing Program Analyst (LPA) Ana Rodriguez met with Lead Teacher Annie Blakey, who granted access to the facility. The purpose of the inspection was to conduct an unannounced case management inspection for a UIR received at Palmdale RO on 3/24/26. LPA disclosed the purpose of the inspection. Upon arrival, there were a total of 8 preschool children in care and 3 staff members present. Description of incident: On 03/23/26, during recess, C2 was following C1 and they fell, C2 landed on top of C1. C1 obtained a crescent shaped wound on their back. LPA Rodriguez interviewed staff present at the time of the incident, children, and parents. A follow up case management - incident inspection will be conducted at a later date to obtain additional information. An exit interview was conducted, and a copy of this report was read and provided to Lead Teacher, along with a copy of the Notice of Site Visit.
