What the inspector wrote

On 04/24/2026, Licensing Program Analyst (LPA) Ana Rodriguez met with Lead Teacher Annie Blakey, who granted access to the facility. The purpose of the inspection was to conduct an unannounced case management inspection for a UIR received at Palmdale RO on 3/24/26. LPA disclosed the purpose of the inspection. Upon arrival, there were a total of 9 preschool children in care and 4 staff members present. Description of incident: On 03/23/26, during recess, C2 was following C1 and they fell, C2 landed on top of C1. C1 obtained a crescent shaped wound on their back which broke skin. There was blood and bruising present. During the visit, LPA Rodriguez conducted a tour of the facility and conducted interviews with children, staff, and parents. Per interviews, the incident was communicated to parents. Per parents request C1 no longer attends the facility. Staff increased supervision and no longer take their breaks during outside play time. Staff reports that C2 has not caused any further injuries to others. LPA determined no further follow-up is needed. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted and the report was reviewed with facility representative, Annie Blakey.
