On 10/10/2025, Licensing Program Analyst (LPA) Hanna Cha met with Site Supervisor Belinda Tackett and Administrator Kathleen Duncan to conduct a Case Management Inspection for the purpose of following up on an unusual incident that was reported to the Department on 10/06/2025, concerning child on child inappropriate contact. Upon arrival at the facility, LPA observed seven children with three staff providing care and supervision. Teacher-child ratios were observed. Description of the Incident: On or about 10/02/2025, Child #1 stated having inappropriate contact with Child #2 while in the bathroom. During this inspection, LPA Cha toured the facility, reviewed files and related documents, conducted interviews with Staff #1, Staff #2, Staff #3, subsequent interview with Parent #1, and collateral interview with Child #1. Based on LPA observations, further follow-up is needed. A notice of site visit was given and must be posted for 30 days. An exit interview was conducted, and a copy of this report was provided to Administrator Kathleen Duncan.
