On 12/12/2025, Licensing Program Analyst (LPA) Hanna Cha met with Site Supervisor Belinda Tackett to conduct a subsequent Case Management Inspection for the purpose of following up on an unusual incident that was reported to the Department on 10/06/2025, concerning child on child inappropriate contact. LPA observed 12 children with three staff providing care and supervision. Description of the Incident: On or about 10/02/2025, Child #1 stated having inappropriate contact with Child #2 while in the bathroom. During this inspection, LPA Cha conducted a safety walk, conducted interviews with Staff #1 and Staff #4, and conducted children interviews. Based on LPA observations, no further follow-up is needed. A notice of site visit was given and must be posted for 30 days. Exit interview conducted and report was reviewed with the Site Supervisor, Belinda Tackett.
