What the inspector wrote

On 09/26/2025, Licensing Program Analyst (LPA) Octavia Nolan conducted an unannounced case management incident inspection at the facility and met with Site Supervisor, Jennifer Cisneros. LPA took a tour of the facility and a census. On 08/29/2025, the facility notified the Community Care Licensing Office (CCL) via telephone about an unusual incident. On 08/29/2025 at 9:25 AM, the children were sitting at a table and cutting paper with scissors. Child #1 (C1) cut Child #2\342\200\231s (C2) ear, causing a 1 cm cut to the area. Staff #1 (S1) washed the area with soap and water. Gauze was applied to stop the bleeding and the child was given an ice pack. Child #2 was picked up by their authorized representative and taken to Accelerated Urgent Care where they were treated with three sutures. The child has returned to care with no restrictions. LPA conducted interviews, reviewed records, and completed observations in the classroom. LPA inspected the scissors the children were using during the incident and verified the tools are designed for children\342\200\231s use. Staff #1 (S1) showed LPA the scissors have been placed in an inaccessible area and children now only cut in groups of three for increased supervision. This appears to be an isolated incident and staff took appropriate measures to address the child\342\200\231s injury. The facility also followed appropriate reporting requirements as specified by CCL. Exit interview conducted and report was reviewed with Site Supervisor Jennifer Cisneros. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
