Case management deficiency inspection following unusual incident report from December 6, 2023 regarding December 5 incident. Staff forcefully grabbed child's forearm and raised voice at child multiple times. Type B citation issued based on interviews and video review.

Observations

  • Staff 1 forcefully grabbed Child 1's right forearm when child was not following verbal directions
  • Staff 1 raising voice at Child 1 observed at least two times prior to physical incident
