What the inspector wrote

On November 12, 2025, Licensing Program Analyst (LPA) Isabel Ortega met with Facility Director Megan Hall-Silveira and conduct an unannounced a continuation case management inspection. The purpose of this case management is to follow up on a self reported unusual incident report (UIR) submitted to the Department on 08/28/2025. The unusual incident report is regarding a Staff #1 leaving child #1 on a soiled diaper for approximately 20 minutes. Upon arrival, there were 19 children observed in care and 8 staff providing care and supervision. During this inspection LPA was provided with the facility roster, Staff statements from staff #2 and # Staff #3 and a copy of notification of incident report was provided to parent. Interviews conducted and staff statements disclosed staff #1 was observed knowingly leaving child #1 in a soil diaper for approximately 20 minutes. Facility is being cited a Type B violation under personal rights. Facility shall post the notice of site visit visible for 30 days. An exit interview was conducted with Director and appeal right with report and Notice of site visit was provided today.

This requirement was not met by evidence based... On interviews, interviews conducted did disclosed Staff #1 knowingly left child #1 in a soil diaper for approximately 20 minutes. This poses a Health and Safety potential health risk to children in care.
