On May 27, 2026, Licensing Program Analysts (LPA) Valentin Hernandez conducted an unannounced case management incident inspection. LPA Hernandez met with Site Supervisor Adriana Montelongo, toured the facility inside and outside, and took a census. LPA informed the Site Supervisor the purpose of the inspection. On April 23, 2026, the Fresno South Regional Office received a report regarding an incident that occurred the same day. During outdoor activities, Child #1 was running and carrying a toy when she tripped and fell. Child #1 hurt the left side temple/eye, and it became swollen. The child’s mother was immediately notified through parent phone application. Three staff were in different areas of the play yard and a census of 14 children. Site supervisor reported that she saw when child fell. Staff applied ice pack as first aid. Mother picked up the child. Staff reported they will continue to supervise children closely. LPA inspected the area and did not observe any hazards. This appears to be an isolated incident. Staff took appropriate action in response to the child’s injury and followed all applicable policies, regulations, and reporting requirements. Site supervisor will resubmit LIC 624 and include more details of this incident. The report was reviewed, and an exit interview was conducted with Site Supervisor Adriana Montelongo. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiency was cited during today’s inspection.
