What the inspector wrote

On November 13, 2025, Licensing Program Analyst (LPA) Crystal Ali met with Site Supervisor, Jazmin de la Cruz Duarte to conduct an unannounced case management inspection. The purpose of the case management (CM) was to follow up on unusual incident report (UIR) received 11/3/25. Incident began on 10/17/25, 10/27/25, and 11/3/25. Each date one child showed confirmed cause of infectious disease (total of three confirmed cases). Parents kept their children out of school for approximately a week. Center followed cleaning process procedures. There has been no further outbreak of this infectious disease, and all three children have returned to the center. Upon arrival, LPA observed 18 preschool and 3 staff providing care and supervision. During this inspection LPA interviewed the site supervisor, teacher, and toured the entire facility. LPA obtained a copy of LIC 9040, documentation and photos of cleaning process and products. CM findings show that the facility followed Title 22 of the California Code of Regulations and/or Health & Safety codes for reporting incidents and facility cleanliness. No deficiencies have been cited. Exit interview conducted with Jazmin de la Cruz and Notice of Site Visit. Notice of site visit must remain posted for 30 days.
