What the inspector wrote

On Tuesday, May 06, 2026, Licensing Program Analyst (LPA) Isabel Ortega met with Director, Candace Monette to conduct an unannounced case management inspection. The purpose of this inspection is to follow up on a self reported unusual incident report (UIR) that occurred during transition to nap time on 4/21/2026 at approximately 11:35 a.m. This UIR was reported to the Palmdale Regional office(Officer on Duty) on 4/21/2026. At the time of the incident there were eight children in care and one staff providing care and supervision. The unusual incident report(UIR) is regarding Child #1 tripping while carrying personal belongings in a bag with a handle, which was brought from home. According to interviews conducted child #1 was walking quickly holding the bag by the handle, child #1 tripped and fell forward making contact with the edge of the door metal rail-frame, resulting in a single laceration on the left side of the of the head. Facility provided First Aid by cleaning, applying an ice pack, and pressure on wound. Parents were notified in a timely manner and picked up child #1 from facility. Parents transported child #1 to the emergency room for observation and medical treatment. Child #1 received four staples. Child #1 is reported to be doing well, staples have been removed and child #1 is back at care. According to interviews conducted, observation, and file review, it has been determined staff #1 was standing by the door supervising the children as the children were lining up by the door for transition and witness the incident but according to staff #1 she unable to reach child in time to try to prevent child from falling. Facility will not be cited today. Facility has modified transition routines and one additional staff is assisting Staff #1 during nap transition. Additional reminder will be given for safety during transition. Exit interview conducted copy of this report with notice of site visit and appeal rights were provided. Facility must post the Notice of site visit for 30 days visible to parents.
