What the inspector wrote

On 06/27/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Lead Teacher Sandra Flores Nieves to conduct an unannounced case management incident inspection. The purpose of the case management was to follow up on unusual incident report (UIR) received at the Palmdale Regional Office on 06/05/25. Upon arrival, LPA observed 14 preschool children and 3 staff members(1 lead and two assistant teachers) providing care and supervision. Description of incident: On 05/07/2025, at 10:35 A.M., child #1 was climbing down the outdoor play structure when child #1 fell and hit their left side of their head on the handrail. Child #1 sustained a cut about an inch long. Site Supervisor Maria Villanueva immediately cleaned the affected area, applied ice, and comforted child #1 During this inspection LPA interviewed child #1, other children who witnessed the incident, and staff involved. During interviews conducted, it was revealed staff advised for the child to not hang on the playground structure rails, however staff did not intervene or intercept the child from hanging on the rails, which resulted in child #1 falling and sustaining a one inch laceration, which needed adhesive glue from child's physician. Facility will conduct a staff training regarding Children's Personal Rights in Child Care for Plan of Correction: The training consists of online or can be conducted through an outside vendor. Online training can be view by visiting CCLD website at https://ccld.childcarevideos.org . Staff sign in sheets will be completed and sent to LPA Tamayo no later than 07/11/2025 for proof of completion of the Personal Rights Training. Exit interview conducted with Sandra Flores Nieves, along with her appeal rights, and Notice of Site Visit. Notice of site visit must remain posted for 30 days.
